How quickly can the filing be done?

Often the same day the policy is issued. What usually delays reinstatement is not the filing itself but the state's processing time on its end.

What happens if my policy lapses?

Your insurer is required to notify the state, which typically suspends your licence again and can restart the filing period. Keeping the policy continuously in force is the entire point of the requirement.

Is an SR-22 the same as an FR-44?

No. A handful of states use an FR-44 instead, which requires substantially higher liability limits than an SR-22. Which one applies depends on your state and your violation.

Will every insurance company file one?

No. Some carriers decline to write drivers who need an SR-22 at all. Finding the ones that will, at a reasonable price, is most of the work.
